This is a working demonstration of a referral-source market map: every hospice, skilled nursing facility, home health agency, hospital and senior community within ten miles of a point in Pasadena, assembled from public federal and state files and plotted on one map.

The pipeline builds a ZIP ring from the 2024 Census gazetteer, joins CMS Care Compare provider files on the certification number, weights facilities by reported discharge volume, and geocodes each address with a ZIP-centroid fallback where no street match exists. Everything on the map comes from those public sources.

What is deliberately not here. The real version of this artifact is built for a specific operator and carries things that belong to them: a written brief addressed to their situation, scored tiers, per-organisation commercial notes, and layers listing individual care managers and fiduciaries by name. All of it is removed from this copy. No individual is named, no organisation is scored or ranked, and the subject agency is fictional.
